(A) To be filled up by ships whenever a man is discharged after a period of service of not less than 3 months, by school and vessels attached to A/SE. after 1 months of service.
(B) The terms "V.G.I.", "V.G.", "Good", & "Fair" are to be employed in assessing abilities, experience is to be expressed in months, e.g. V.G.I/10, V.G/8, Good/4, Fair/9
(C) Columns 5, 6, 9 & 10 are only to be filled up for ratings S.D. 1st. Class and above. When assessment is made in columns 6 & 8, the A/S device (Loops, Hydrophones, Echo Sounding Gear etc.) is to be indicated in column 12.
